- 表重命名: pay_* → pay_md_* (8张主数据表)
- 启停功能: PayProvider/PayProduct 增加 enabled 字段及 switch 接口
- PayProductConfig 移除 enabled 字段
- PayProvider 增加 description 字段
- SQL: update-tables.sql 完整迁移脚本
